We are now giving families the option to arrange a cremation from the comfort of their home. With this option, the next of kin simply fills out two online documents and gives us a call upon completion.
If a family decides to arrange an online cremation as opposed to the traditional in-facility arrangement conference, we have attached the below documents that will allow us to assist them. The next of kin will never have to set foot inside of our facility.
Affordable Online Cremation - $1195.00
This price includes:
Removal from place of death, filing of death certificate, basic cremation container, cremation process, online obituary notice, and the return of the cremains in temporary urn via mail.
Cash advances are not included in the Affordable Online Cremation price.

Upon completion of the above documents, scan and fax (904) 259-4601 or email (forbesfuneralhome@yahoo.com) the documents, as well as a copy of the next of kin's photo I.D.
If your loved one was a member of the military and received an honorary discharge, attach a copy of their discharge paperwork (DD214) and we will obtain a burial flag for you.
